\nOSF HealthCare Neuroscience Institute is seeking an Advanced Practice Provider to join our growing Stroke and Cerebrovascular Disease Program. This unique opportunity allows an APP to work alongside a dedicated Stroke Neurologist in building a comprehensive outpatient stroke program focused on post-hospital transitions, secondary stroke prevention, vascular risk reduction, and long-term cerebrovascular care.
\nThe APP will practice exclusively within the Hospital Outpatient Department (HOD) and serve as a key member of a multidisciplinary neuroscience team committed to improving access, continuity of care, and outcomes for stroke patients throughout Central Illinois and surrounding underserved communities.

\nPosition Overview
\nThe Stroke APP will work closely with a Stroke Neurologist who provides care across the inpatient and outpatient continuum. The APP will help ensure a seamless transition from hospitalization to outpatient follow-up, providing timely access for patients following stroke, TIA, and other cerebrovascular events.
\nThe position is focused on:
\nThe APP will play a vital role in reducing readmissions, improving patient outcomes, enhancing continuity of care, and expanding access to specialty stroke services.
